Policies

FLORIDA DEPARTMENT OF HEALTH COMPLIANCE*

To adhere to Florida Department of Health regulations, every patient must maintain an accurate Patient Profile, which includes:

 

- First Name

- Last Name

- Date of Birth

- Phone Number

- Email Address

- A valid Credit Card on record.

 

Patients must personally coordinate their appointments, ensuring the integrity of their medical records. Booking for others or transferring appointments is not permitted.

 

**DEPOSIT POLICY**

 

A non-refundable deposit of $50 is mandatory for each service when booking to confirm your slot with the provider.

 

**RESCHEDULING AND CANCELLATION**

 

- Appointments should be rescheduled a minimum of 24 hours ahead.

- To retain your $50 deposit, reschedule rather than cancel.

- Deposits are only transferable to a rescheduled appointment.

- A maximum of three reschedules are allowed per appointment.

If phoning, leave a detailed message with your full name and appointment date. Also, text us for further communication.

 

We prioritize in-office patients and might not always be available on the phone. If unanswered, please leave a voicemail. Emergency messages are prioritized. For other matters, expect a response within one business day (M-F, 9:30 am - 4:00 pm). Remember, the Patient Portal is accessible 24/7 for appointment management, form submissions, and more.

 

**NO-SHOW POLICY**

 

Failure to cancel or reschedule 24 hours before the appointment incurs an additional $75 fee, in addition to the $50 deposit.

 

**APPOINTMENT CONFIRMATIONS**

 

Patients receive confirmations via email and text 3 days and 26 hours before their appointment. It's crucial to monitor and respond timely to these notifications to avoid extra charges. Late acknowledgments won't be considered.

 

**LATE ARRIVALS**

 

For Tox appointments (15 min duration), a 5-minute grace period is allowed. For Filler appointments (30 min duration), it's 10 minutes. Exceeding these limits results in a $75 no-show fee. If you're late but the provider can still accommodate, not all services might be available, and the late fee won't be deducted from your balance. If you're too late and the provider can't see you, any deposits for that slot will be forfeited.

 

**BOOKING FUTURE SLOTS**

 

Patients can book multiple future appointments. A deposit is necessary for each scheduled service.
